How does the way you ask a question alleviate or exacerbate maths anxiety?

I haven't found this to be an issue with one-to-one teaching, as I can quickly identify any anxiety and so rephrase the question or reassure the student. However, when teaching in a class, this isn't as easy. An English teaching friend advised me to let anxious students know what questions I will be asking of them. This gives them time to think about the subject (and to focus - always a bonus ). I found this worked well. Whereas good questioning techniques are essential, our response to an incorrect answer is so important, too.

Yes, for some students I think it does. I have noticed that for students who have high maths anxiety the opening words can influence how they interpret the question, comment or feedback. While we need to build resilience, some students may need extra thought put into teacher wording.
